Assessment of the State of the Art of SBRT in Prostate Cancer in Latin America: The HUG-Clinical Working Group Initiative

Objectives: : To assess the use of SBRT technique in prostate cancer among Halcyon (Varian) platform users in Latin America. The purpose of this survey was to define the current state of clinical practice, identify areas for standardization, and establish a continuous education program aimed at improving treatment quality and structured data collection, impacting real-world data (RWD) and real-world evidence (RWE)-based practices.

Methods: : A questionnaire was distributed to 40 radiation oncologists (RadOncs) from Latin America, all users of the Halcyon platform, via the Forms platform. The survey evaluated the use of SBRT in prostate cancer, the implementation of protocols, SBRT techniques, and the level of experience in the field. Data were collected on the application of SBRT across different institutions

Results: : Thirty-five centers from 11 Latin American countries were covered: Brazil (35%), Colombia (22.5%), Argentina (15%), and Chile, Mexico, & Uruguay (5%). Of the respondents, 45% indicated that they use SBRT in the treatment of prostate cancer, with 45% using doses of 36.25 Gy in 5 fractions, showing a slight tendency to increase the dose with higher Gleason scores. Seventy-two point five percent (72.5%) of participants applied a dose limit for the urethra, while only 25% did so for the Vesical Trigone. Forty-seven point five percent (47.5%) used the PACE-B trial as a reference for defining OAR dose limits.

Conclusion(s):: The variability in SBRT practices in Latin America highlights the need for standardization and structured data collection to improve treatment quality in the region.
The HUG-CWG initiative on Prostate SBRT is developing a continuous education program to standardize and protocolize quality levels in the region, with the support of Varian Medical Systems.
